Your child’s autism history – get my FREE document to help you with all the information that you need to share with your child’s team.

As a mum of an autistic child, I know that you know you’ve lost count of how many appointments you’ve already attended.  Paediatrician, Speech, OT, Psych, GP, Audiologist, Optometrist, Behavioural Paediatrician.  The list literally goes on and on.

And during those appointments how often were you asked that same question – Ok tell me a little more about your child.  What is his or her history, when was he/she diagnosed, who made the diagnosis, etc.

After that many appointments, I literally got sick of saying the same thing over and over again, and the problem here is that its not a small amount of information.  I had to recount dates, ages, names of doctors, symptoms, etc.  It’s a lot to remember when you’ve already got so much on your plate –  trying to remember facts, potentially settling your dysregulated child and ensuring you keep track of the time.

So, what’s the solution to this problem?   I created a document where I would write down everything that had happened up until the current day.  As time went on, I would update it when things changed or something happened.

Included in this document were things such as:

– What year was your child diagnosed.

– By whom was he diagnosed.

– Hold old was your child when he was diagnosed.

– What were the tell tale signs that you, the parent, picked up that you felt were red flags.

– When did your child start speech therapy

– Who is/was your speech pathologist.

– When did you child start occupational therapy.

– Who is/was your occupational therapist.

And so much more.

I want to share the form that I used to record all this information so that you can use it as well.  CLICK HERE to get your free copy of My Child’s History Form.

The reason I found that this form was so relevant:

– I could send the form to the relevant person – GP, Paediatrician, Speech, OT, literally anyone who was going to be a relevant person in my son’s life when it came to his autism and they were completely informed.

– It saved me so much time in the appointment which meant I had more time to ask the questions that I needed to ask.

– It saved me time and energy from having to repeat it every single time.

– It stopped me from becoming triggered because I wouldn’t have to bring up old wounds.

– I didn’t have to wonder if I got the information correct.  Sometimes when we have so much on our minds, we can get information wrong.

So, as you can see, this form was completely and utterly invaluable!!  Some of my clients have already started using it and are loving it.

I hope that you download the History Form HERE and use it as well, it will save you so much time and energy and allow for things to be much more streamlined.
